〈過ぎたるは及ばざるが如し〉

昨年の原発事故の後、放射能の影響を少なくするには、ヨウ素を摂るといいのではないかと言うことで、ヨウ素をたくさん含んでいる海藻を毎日食べると言う人がいました。
しかし、「閉経後の女性が海藻を食べ過ぎると甲状腺癌のリスクが増す」、と言うニュースがありましたし、閉経後の女性でなくとも、もともと甲状腺に疾患を持っている人が、海藻を摂りすぎると、甲状腺機能に異常が起こることがあったりします。
ヨウ素は人間の体にとって必要なものですが、摂りすりはかえって体に悪影響をもたらすことがあります。日本は海に囲まれており、普通の食生活でも海産物からqヨウ素は充分に摂取していますし、もともとが海底であったため、大陸の内部や高知に比べると土壌に� �ヨウ素が多く含まれており、野菜などからもヨウ素が摂取できますので、あまり意識して海藻を食べなくても充分なわけです。
ビタミンＡやＣも体に必要なものですが、摂りすぎるとかえって悪影響が出ることがあります。これらは普通の食生活で体に悪影響が出るほど摂りすぎることはまずありません、どちらかと言えば足りないことを心配することが多いでしょう。
しかし、ビタミン不足を気にして、ビタミン剤やサプリメントで不足分を補おうとすると、過剰摂取してしまうことがあります。
何事も、「過ぎたるは及ばざるが如し」、と言うことになるでしょうか。ビタミンやミネラルはバランスのよい食事から摂り、ビタミン剤やサプリメントは補助的なものにしておくのがよいようです。

他分野から見た獣医学教育のあり方

（以下は、平成12年4月6日につくば国際会議場において開催されたシンポジウムのうち、３題の講演の記録である。この他に、京都大学の加藤尚武氏による「人と動物の関係-倫理学の立場から-」および、関東第一サービスの小野寺威氏による「これからの獣医学教育に望まれるもの-製薬企業の立場から-」を加えて、5題の講演が行われた。）

公衆衛生分野における獣医学教育の課題

現代社会において、人々が「動物」との接する機会はますます多様化している。その形態は、単に動物に直接接触することのみならず、間接に動物を利用しその恩恵を社会が受けていることでもあり、また、そのなかでも、例えば、前者では、従来であれば家畜飼養であり今日ではペットなどを中心とした愛玩動物、また将来的には介護犬などであり、後者では食肉、牛乳、卵など動物性食品の利用のほか、実験動物などである。このような機会は私たちに多くの利益や喜びを与えるものであるが、−方では、何らかの原因により、これらの新たな関係からは、時として弊害やそれに伴う社会的不安も生まれ出ている。

例えば、いわゆる人畜共通伝染病のように、従来からその疾病について知られているものにおいても、利用する動物種やその調達先、利用法などの変化により、これまでとは違った形でその姿をあらわす場合がある。また、従来から知られていない全く新しい疾病も報告される状況にある。これらは、いわゆる新興・再興感染症と呼ばれ、それらの疾病の台頭は社会構造の変化などを含めて新たな疾病に対する調査研究や対策が求められている。

従来、獣医師は、家畜の診療を中心に、ヒトが動物に直接的に接し利用する場において、その専門知識を持って動物の疾病治療を中心に行ってきたが、今日では、単に動物と人が接する機会以外のより広範囲に獣医師の活躍の場が広がりつつある。例えば、公衆衛生分野は、食肉など食品衛生のほか人畜共通伝染病などの対策において、獣医師が活躍しまた今後活躍を期待される分野ではあるが、そこでは従来にも増した専門知識とより広範な実践的技術が求められている。

公衆衛生分野における獣医学への期待

１）感染症対策

人に対する感染症のなかには、狂犬病のように動物を主体とし人に対し重篤な影響を及ぼすものがある。これらの疾病については、従来から獣医師により動物に対する直接的な管理とともに、動物の登録や予防接種などについて法律の整備など公衆衛生上の対応が取られている。一方、近年、ラッサ熱やエボラ出血熱のようなウィルス性出血熱など新たなに人に対する感染症が報告されている。これらの７０年代後半から報告されはじめた感染症は、新輿・再興感染症と呼ばれ、それらのなかには動物由来感染症も多い。我が国の感染症対策では、平成１１年４月に「感染症の予防と感染症の患者のための医療に関する法律」（いわゆる感染症新法。対象となる疾病及び疾病の概要は図１のとおり）を施行し、感染症対策の一環とし� ��動物由来感染症対策を進めており、動物由来感染症対策の強化が強く求められている。

２）動物性食品を中心とした食品衛生対策

動物性食品を中心とした食品衛生対策では、従来から食肉や牛乳、またそれらを利用した製品の安全確保について獣医師が活躍していきた分野である。食中毒の発生状況(図2及び表1)では、今日でも従来と同様に、サルモネラや病原性大腸菌などの微生物により発生するものは報告されるものの多数を占め、また、その原因となっている食品は動物に由来するものであることが多い。これらの食中毒に対しては、従来からサルモネラなどの食中毒原因菌と主な原因食品を対象にと畜場における衛生対策や食品の製造基準等を施行し、食品衛生対策を進めている。

一方、従来からの食中毒に加え、近年、狂牛病やいわゆる耐性菌の出現など畜産形態の変化を背景とする問題が顕在化し始めている。これらの問題は、単に畜産形態が集約化されたことによることのみならず、動物性タンパクの再利用や動物用医薬品の利用などの新たな技術の応用にあたって出現したものである。

それらの対策にあたって、例えば、狂牛病に関しては、現代の科学でもその病因物質を特定するには至っていないものの、動物性タンパクの再利用の制限など畜産形態の変更により対策が講じられている。このほか食品衛生法分野では、我が国では魚など魚介額の消費が多いが、貝毒など有毒魚介類など水産食品衛生対策も重要になっている。また、有機農法等の新興により畜産廃棄物由来に由来する食品汚染も懸念されている。

３）実験動物利用への貢献

実験動物の利用は、化学物質に関する毒性評価、疾患モデルの開発など広範にわたり行われている。環境中に排出される汚染物質や工業分野において使用される化学物質による人への健康影響の評価においては、実験動物を利用した毒性評価は必須であり、また、最近ではいわゆる環境ホルモン様作用に関して知見が集まりつつある状況にあり、より微量で微少な影響に関する毒性評価試験の実施が求められている。このような状況は、実験動物の長期間にわたる健康菅理をはじめ、被験動物の観察や病理診断などが重要になっている。

また、近年の急速な高齢化社会の到来により、生活習慣病をはじめ種々の慢性疾患などについて予防をはじめ治療法の開発などが求められる状況にある。これらのことから、実験動物の活用については、今後機能に着目した評価などより広範な需要が見込まれているものと思われる。

４）基礎医学研究への貢献

人工授精などの繁殖工学では獣医学領域での応用が進んでいるが、これらの技術の人への応用など活用範囲が拡大している。また、バイオテクノロジーの応用により、生物由来の製剤や生物学的組織など動物の利用を伴う技術開発などが進んでいる。これらの技術の利用にあたっては、獣医学領域で蓄積される知見の利用が期待される。

５）その他

環境ホルモンなどの環境衛生対策、社会福祉分野での愛玩動物利用などの分野で活躍が期待されている。

獣医公衆衛生学教育への提言

以上、述べてきたような公衆衛生分野での獣医師の活躍に対する需要や期待に対応するにあたっては、大学における獣医師養成のための専門教育においてこれらの専門分野に関する教育の充実が期待されるところである。そのアプローチとして、いくつかの連携が考えられる。例えば、実社会での二一ズと現在の教育内容の比較するとか、海外での教育内容との比較、また、世代間や地域間での交流も教育内容について多く示唆を示してくれるものと期待される。

これら公衆衛生分野での多様な期待に対応するため、獣医系大学ではこれらの分野の基礎を網羅する教育が求められている。そのため、獣医学系大学間での協カのみならず、内外の獣医系以外の教育研究機閨、公衆衛生行政機関との連携や交流を促進することが必要であると考える。また、獣医師に対する卒後教育も重要であろう。

特に公衆衛生分野では、専門分野での追加的な教育が必要であることから、例えば、大学教育を終了した者や公衆衛生分野で実務を担当する者を対象に、公衆衛生学修士課程での教育訓練が必要である。公衆衛生学修士課程は、海外ではスクール・オブ・パブリックヘルスとして定着している教育課程であり、我が国でも国立公衆衛生院が対応している。しかしながら、獣医学系を中心とした公衆衛生分野での高等教育が必要である。

私は１５年のブランクの後、経験の無いクリニックの外来に復職しました。

初めてな事だらけの外来業務を、覚えてこなすのにいっぱいいっぱいですが、診療の流れを止める事はできず、採血・注射・点滴・処置・心電図・レントゲンセッティングなど、こなさなければいけない業務が次から次へとくると、慣れない上にしっかり頭に入っていない、その状況で仕事をするので焦るしひどい時は頭が真っ白パニック状態になり緊張で汗が出ます。

私の仕事っぷりがひどい時はパートナーのナースの罵声もあるので、それにもビビってしまい、完全に萎縮してしまいます。

脳内出血（のうないしゅっけつ）とは、脳内に出血する疾患である。

大きくは高血圧性脳内出血と、非高血圧性脳内出血に分類される。

脳内出血は、寒冷暴露などの自然環境のほか、労働条件やストレスなどの社会的、精神的要因がある。また、喫煙、塩分摂取、アルコールなどの嗜好、肥満、高血圧、運動不足など多岐にわたる。低コレステロール、低中性脂肪もリスク要因である^[1]。

高血圧性脳内出血は、高血圧症および動脈硬化が起こる50～70歳台に多いとされるが、近年高血圧症の早期治療の普及により減少傾向にある。他の危険因子として喫煙、糖尿病、動脈硬化症、種々の出血性疾患がある。

死亡率は75%に達するとも言われる。平成16年度厚生労働省人口動態統計では、人口10万人対で本症による死亡が28.6人であった。

出血部位により、被殻出血、視床出血、皮質下出血、脳幹出血、小脳出血に更に細分化され、発症部位により症状は異なる。

非高血圧性脳内出血は、脳動脈瘤、もやもや病、脳動静脈奇形、脳アミロイド血管障害（脳アミロイドアンギオパチー）、脳腫瘍内出血、抗凝固療法に合併するもの、アンフェタミン乱用に伴うもの、血小板機能障害に伴うものなどがある。老人においては脳アミロイド血管障害による脳出血は非常に多く、高血圧性につぐ第二位である。脳アミロイド血管障害では皮質下出血が多く、また再発を繰り返すことが多い。

出血部位によって分類するのが一般的である。内出血は外科的な治療の適応となることが多い。基本的に行われる手術は血腫除去術であり、その意義は救命のための脳ヘルニアの回避、圧迫脳の減圧、出血源の特定と止血にあるとされている。出血の大きさ、神経症状、全身合併症、年齢、術前ADLによって手術適応、様式、手術時期を検討する。一般的に血腫量が10ml未満の小出血または神経学的な所見が軽症の場合は部位に関係なく手術適応はなく、また意識レベルが深昏睡（GCS≦4）の症例も手術適応はない、

多発性硬化症の基礎知識と療養のポイント

１ 多発性硬化症（MS）とはどんな病気？

脳や脊髄の神経細胞には、軸索（じくさく）と呼ばれる突起があり、この突起が他の神経細胞につながり、細胞と細胞の間で情報の伝達を行っています。軸索には、それを包む 【さや】のようなものがあり、髄鞘（ずいしょう）と呼ばれます。髄鞘は、突起を保護 したり、電気的な情報の伝達をスムーズに行うような働きをしています。この髄鞘が、炎 症により壊されることを脱髄（だつずい）と言います。脱髄が生じると、神経細胞の情報 伝達がうまくいかず、麻痺やしびれをきたします。多発性硬化症は、脳や脊髄などの中枢神経に脱髄をきたす疾患です。炎症により、脱髄が生じる詳しいメカニズムはまだ分っていませんが、免疫の異常が想定されています。

（１）どのくらい患者さんがおられるのでしょうか
日本での患者数は、人口10万人あたり3〜5人ほどであり、女性に多くみられます。
全国で約5,000人、日本でも緯度が高くなるほど患者数が増えます（緯度効果）。
約80％が15〜50歳で発症するといわれています。ピークは30歳頃、家族性はごくわずかで約1%位です。

（２）多発性硬化症には、どのような症状がありますか
（初発症状）
ほとんどが急性に発症し、一週間以内にピークに達します。自然に改善することもあります。視神経、脊髄、大脳などの病巣により症状も異なりますが、視力障害、しびれ感、運動麻痺、歩行障害などが多く出現します。

日本では視神経、脊髄の障害が多く現れます。

第五章　にきびの種類は様々　
尋常性座瘡
尋常性座瘡とは、いわゆる我々が「にきび」と呼んでいるものの正式名称です。
尋常性座瘡は主に10代に発症するにきびですが、大人になるまで症状が続いたり、大人になるに連れて悪化したりする場合もあります。
尋常性座瘡が軽度のものである場合、以下の様な症状が出ます。

ホワイトヘッド
毛穴が完全に塞がれると、皮脂やバクテリア、死んだ皮膚細胞は閉じ込められ、皮膚の表面に白いものができます。
これがホワイトヘッドですが、ホワイトヘッドはブラックヘッドに比べて寿命が短いため治りやすい症状です。

ブラックヘッド
毛穴が部分的に塞がれた状態になると、閉じ込められた皮脂やバクテリア、
死んだ皮膚細胞が徐々に皮膚の表面に排出されブラックヘッドとなります。
見た目が黒いのは汚れによるものではなく、皮膚の色素であるメラニンが空気中で酸化して起こるものです。
ホワイトヘッドは寿命が短いのに対して、ブラックヘッドはよりしっかりとした構造を持っており治療には時間がかかります。

小丘疹
小丘疹とはヘッドが無い、やわらかい出来物で、炎症を起こして赤い状態になります。

膿疱
膿疱はホワイトヘッドに似た特徴を持っていますが、ホワイトヘッドのように炎症は起こしません。
赤く丸い出来物で、真ん中が白か黄色をしています。
これが俗に「にきび」と呼ばれる最も一般的なにきびです。
にきびが重症の尋常性座瘡であるかどうかは、次の小節や嚢胞があるかないかで分かれます。

現代人の多くが肩こりで悩んでいて、肩こりは今や子供たちの間にも増加してきています。

肩こりのタイプは様々で、片側だけこるタイプ、首から後頭部まで広がるタイプ、肩甲骨を中心にこるタイプ、腕までの広範囲に及ぶタイプなどがあります。

症状も重感、だるい、しびれ、痛みなど様々です。

こりという現象は、乳酸、ピルビン酸などの疲労物質が体内に増加して、筋肉細胞のタンパク質と結合して硬くなった状態です。

初期には、だるさ、重さなど疲労感として感じますが、長く続くと炎症にまで発展して、痛みを感じるようになります。

血液のねばりが増したり、体内のうっ滞のため、血行が阻害されてしびれることもあります。

脳脊髄液減少症の英訳は「Ｃｅｒｅｂｒｏｓｐｉｎａｌ　Ｆｌｕｉｄ　Ｈｙｐｏｖｏｌｅｍｉａ」略して「ＣＦＨ」と呼ばれています。

目的のホルモンで、尿崩症などの治療薬としても使われています。尿などで水分が体外に排出
されるのを防ぐので血中水分量が増すので血圧が上昇します。

上記二つの理由を考えるといくつか「そうなる原因」が見えてきます。

(1)〜積極的・直接的なニキビ痕の治療〜

２日前と同じ、また夜中の３時頃だったと思う。

寝ていてふと、背中の痛みで目が覚めました。
あー、また痛いなー・・・と思っているうちにどんどんどんどん痛くなってきて、寝てられなくなってきて、リビングに行って痛みを逃すためにウロウロ、座り込んでうめいて、ゴロゴロしてみたり。
あー、吐いたな。吐きもした。
キッチンでうずくまってたらダンナが起きてきて「大丈夫ですか？」と。

本日月曜日。
ダンナは仕事に行かなくてはいけないので、こんな情況でも息子の面倒を一人で見なくてはいけない・・・
つーか、この痛み２度目だけど、尋常じゃないでしょ。
胃薬効かなかったの？胃じゃないの？
うーん、タイムリミットはダンナの仕事が始まるまでか・・・・

こんなコトをぐるぐると考え、「ERお願いします」と、今日は決心しました。

激痛の中、自分のモノよりも息子のモノを率先してさっさと準備。
量としては3~4日分あったと思う。
（ミルクは箱ごとだったし）
とっさのこととはいえ、何か予感することがあったのでしょうか。
搾乳機も、自分の薬も忘れずに準備。
家を出る前に息子にちゃんと授乳もした。
つーか、あの痛みの中よくやったよ自分。
時々痛すぎてその場ダッシュとかしておりました（笑）

1ヶ月ぶりにHuntington Hospital へ。
たしか4時前でした。

ERにまず一人で駆け込んで、症状を説明。
まだこのときは胃だと思っていたので、「あいはばすとろんぐすとまっくぺいん」と説明。
1ヶ月前に出産しただの、下痢はあるかと聞かれて「今この抗生剤を飲んでて、このせいで下痢だ」だの、「授乳のせいでこって背中痛い可能性は？」と聞かれて「そう思わない」と答えたり、自分よく英語とそれ以外での説明を頑張ったと思います。（自画自賛）
血圧と体重を量ったあと、とりあえず座って待っててと言われたので待ってたら、車を停めたダンナと息子が登場。
すぐにERのお部屋に呼ばれ、ガウンに着替えてベッドに横になるように言われたのだけど・・・・

筋骨格系の痛みに対する科学的な考え方　

ほとんどの頚痛、腰痛、肩痛、膝痛や手足のしびれ（知覚麻痺のことではありませんよ）は生理的トラブルのためです。構造の異常のためではありません。また生理的トラブルが構造の異常でおきているという証拠もありません。

痛みはレントゲンやＭＲＩで表すことのできない生理的なトラブルです。ところがレントゲンやＭＲＩを使ってそれを説明しようとします。ここに大きな問題がひそんでいると考えます。

伝統的医学は構造上の異常が痛みの原因とみなして説明します。多くの医師や患者さんはそれを今だにかたくなに信じています。だから治せない、治らないのです。医師にとっての悲劇は間違った勉強をさせられてきたということです。権威ある立場の医師はいまさら方向転換をしにくいものです。この事実は患者さんにとっても大いなる悲劇です。

生理学で「構造上の異常が痛みをつくる」という根拠(evidence)がありませんから、問い詰めていくと説明がつかなくなっていきます。患者さんには自分にとって不都合なデーターは無視して、都合のよいデーターだけを用いて説明する傾向があります。悪意があるわけではありませんが伝統的な医学を信じて思いこんでいるのです。

軽視してはいけない"めまい"

はじめに

めまいに悩んでいる人は、皆さんの周りを見渡しても少なくないはずで、心筋梗塞や脳卒中になる人よりも多いといわれています。

患者さんがこれほど多いのに、なぜか医学界ではあまり重要視されず、めまいを克服するための積極的な研究や対策もほとんど行われてきませんでした。その理由の一つは多くの医師が「めまいは耳の病気で、放っておいても死ぬようなことはない」と軽く考えているからのようです。

患者さんにとって深刻でうっとうしい症状ですが、耳鼻科では「異常ありません」といわれる場合が多く、「なんとかしてほしい」と病院を転々とする人が少なくありません。実は、めまいはさまざまな原因で起こる症候の総称で、耳鼻科だけですべてが解決するような病気ではないのです。

めまいはある部分では、循環器病と密接に関係していることをよく知っておいてほしいのです。

めまいに二つのタイプ

＜表１＞のようにめまいは、大きく分けて二つのタイプがあります。一つは、自分自身がグルグル回っているように感じる「回転性めまい」で、水平方向に回転しているように感じる場合が大半です。エレベーターに乗ったときのように、すーっと下がっていく感じを訴える患者さんもいますが、これは垂直方向への回転性めまいと考えられています。もう一つは「フラフラする」か「グラグラする」タイプの「非回転性めまい」です。

どちらのめまいであっても、同時に吐き気、嘔吐があることも少なくありません。吐き気や嘔吐はめまいにつきものの症状なのです。

専門的には回転性めまいを『めまい』、非回転性めまいを『めまい感』と呼んで、両方を区別しています。しかし『めまい』と『めまい感』では混乱し、話がわかりにくくなるおそれがあります。そこでここでは、めまいを「回転性めまい」、めまい感の方を「フラフラ感」と呼ぶことにします。

回転性めまいは、突然起こって、ある一定期間だけ持続し、その後はまったく消えてしまうのが普通です。

一方、フラフラ感には、回転性めまいと同じように突然起こり一定期間だけ続いた後に完全に消えるタイプと、いつの間にかフラフラするようになって、それが何か月も、何年も続くタイプがあります。

ですから、めまいはその起こり方と持続の仕方によって、「突発性一過性のめまい（回転性めまい、フラフラ感）と「慢性持続性のめまい（フラフラ感）」の二種類に分けることもできます。

どこの病気で起こる？

めまいは一体どこの病気なのでしょう？めまいの起こる仕組みはかなり複雑で、耳、眼、首、脳などいろいろな部位の異常が関係している場合があります。

めまいを理解するには、まず人が頭の位置や、身体の向きをどうやって認識しているかを知る必要があります。なぜなら、頭や身体の位置を認知する機構の一部が障害された結果、起こる症状がめまいだからです。

人の身体には＜図１＞をご覧いただくとわかるように、頭や身体の位置を確かめる極めて精巧な仕組みが備わっています。このおかげで、自分の頭や身体がどういう方向を向いているかを知ることができるのです。

この仕組みの最前線で働いているのが眼、耳、手足などの末梢感覚器です。これらの感覚器はそれぞれが得た情報を脳に伝えます。脳に送られてきた多くの情報はすべて最高機関である平衡中枢に送られ、ここで「自分の頭や身体はいま、どの方向を向いているか」を最終的に判断します。

普通、眼や耳、首、手足から送られてくる情報は完全に一致しています。例えば、水平な地面に立っている人が首を45度ぐらい右に傾けて、前方の絵画を鑑賞しているとしましょう。

子供の喘息というのは、発作が起きると、とても苦しそうなので、お父さんやお母さんは、本当に心配になってしまうと思います。何とかして治してあげたいと考えられると思います。例えば、スイミングに通わせているという人も、多いのではないでしょうか。「喘息には水泳が良い」という話は、よく耳にします。なぜ、喘息には水泳が良いのでしょうか。

ここでは、子宮内膜症の基本的なことをまとめていきます。
まず、内膜症が心配な人、市販薬ではダメな下腹部痛のある人は、大きな総合病院に行ってみてください（情報公開が進んでいる国公立がとりあえずいいかも）。内膜症の自覚症状は、ＪＥＭＡサイトの正しい医学と医療のグラフを参考にしてください（月経量が多い、塊が出る、不正出血などは腺筋症の特徴で、ふつうの内膜症とは関係うすい）。性交痛や排便痛は内膜症に特徴的で、下腹部臓器に癒着があることを示しています（ただしクラミジア他の感染症でも癒着はできる）。
内膜症の痛みがどう出てくるかは千差万別で、年を追ってしだいに強くなっていく人、何ともなかったのに突然激痛に襲われて救急搬送になってしまう人と、えらく違います。

[１]子宮内膜症の基本事項～診 断

病院に行って内膜症と臨床診断されても（問診、内診、超音波エコー、ＣＡ－１２５などの血液検査、ＭＲＩなどの、組み合わせ診断）、セカンドオピニオン（別の病院）をとりましょう。初診でＧｎＲＨアゴニストやダナゾールを処方されそうになっても、一度帰って考えると言いましょうね。低用量ピルや黄体ホルモンなら、初診でもらってもいいですが。内膜症を確かに臨床診断できる医師は、内膜症の手術に参加している医師と考えたほうがいいですよ。

ここ10年くらいでしょうか、内膜症と腺筋症はオイシイ病気になってしまいました。薬物治療を半年もさせることができる、薬価は高い、それをしても治るわけでもないので、何年も薬物治療患者としてつないでおける病気だからです。こんな医師につかまってしまうと、時間とお金を浪費するだけでなく、不健康になったという結果がついてきます。
内膜症の確定診断は手術ですが、これは診断としての価値より、治療価値のほうが圧倒的にあります（こんな病気も珍しいでしょうね）。

◆通院は一人で行くのがいいと思うけど･･･

若い人には厳しい話になるかもしれないけど、親や夫や恋人は、内膜症の本人とは存在が別の他者ですから、本人の痛みやしんどさや悩みは、本人と同じには絶対にわからないということを、肝に銘じておいたほうがいいですよ。疼痛が強く長くなってきたときや（仕事や日常生活のＱＯＬが下がってくる）、たとえば不妊状態が１年２年と出てきたときに、まっさきに私たち本人を困らせてくれるのも、親や夫や恋人だったりします。もちろん、支えてくれる親や夫や恋人もいますよ（支え方がうっとうしかったりすることもある）。
とくに母親は、娘の内膜症を過大視する、過小視するの、どちらかに傾いているのがほとんどのようです。また、母親の女性疾患経験が役に立つか邪魔になるかも、難しいところです。さらに、本人のセックスライフや仕事や日常生活のさまざまなことは、母親の知らないことも多いはずだし、一緒に診察室にいると、結局はマイナスなほうが多いように思います。
夫の場合には、不妊医療をするかどうかというあたりからは、一緒に病院に行くべきです。
つきあっている相手は、親や夫以上の他者ですから、結婚（事実婚）の相手でないかぎり（それでも本人だけで行くほうがいい）、一緒にいく価値はほとんどないでしょう。 内膜症は私たち本人の身体問題ですから、本人自身が医療や人生を選択しながら、閉経までつきあっていくものです。
性交痛について、自分では説明できず、彼に医師に説明してほしくなったときなどに、つれていくのはいいと思います。

[２]子宮内膜症の基本事項～治療の基本(薬と手術)

臨床診断の人は、内膜症や腺筋症ではない確立が３割あるので、低用量ピルか黄体ホルモンが基本治療法だと考えてください。これらは、月経困難症（月経痛）、月経後から排卵頃までの下腹部痛、過多月経、ＰＭＳ(月経前症候群)などの、世界標準治療法です（これを知らない医師は結構いる）。

そのなかでも内膜症の確立が高い人は、保存手術が基本治療となります（これが確定診断にもなるだけのこと）。内膜症はがんと似ていて、薬物治療だけでは何ともならない外科疾患と考えてください（ヘタな手術は術後癒着が多いため、痛みがまし、不妊が深まる）。それでも、手術ってなかなか大変な選択ですよね、そういう場合も、低用量ピルで心身を休めてあげて、ゆっくり考える時間をもつことをお勧めします。

保存手術のあと（確定診断にもなる）、再発したら（再発と言えるのは２年以降のことで、それより早いのは取り残しだろう）、もう一度保存手術をするか、準根治手術（子宮摘出）や根治手術（卵巣摘出）をするか、薬物治療をするかに、分かれます。 薬物治療の目的は、排卵を止めることと、女性ホルモン分泌大変動を一定化してやることで、病巣の進み具合を遅らせることです。

その方法として、ＧｎＲＨアゴニストによる偽閉経治療（50代の閉経期から70代の老年期までどこになるか個人差が大きい）、ピルによる偽妊娠治療（妊娠初期状態）、ダナゾール治療（更年期状態）、黄体ホルモン治療があり、ＧｎＲＨアゴニスト以外は内膜症病巣への直接攻撃作用があると言われています（ただし消せるわけではない）。

ですから、慢性疾患である内膜症に対するこれらの効果は同程度と考えて、リスクの違いでそのときどきに選択する必要があります（リスクが最も高いのはＧｎＲＨアゴニスト、最も低いのは黄体ホルモン）。

◆ホルモン剤の基本

ピル、黄体ホルモン、ダナゾール、ＧｎＲＨアゴニスト、この４種類のホルモン剤の効果は同じことで、膨張している卵巣嚢胞がある場合には、それをやや小さくしてくれます。これは、ホルモン剤の使用中と使用後の排卵復活までの間は、排卵がないため、卵巣は自己活動を停止するので、たまっていた血液の水分が自然吸収されるからです（ドロッとしてくる）。嚢胞の病巣は、嚢胞の一部分だけですが（風船を膨らませた時の結び口の部分程度と思ってね）、どんなホルモン剤を使ってもこの小さな病巣は消えることはありません、ちょっと寝ててくれる程度です。つまり、どのホルモン剤を使おうと、排卵が止まり、ホルモン分泌大変動も一定化されるため、病巣や癒着がふつうに成長するのをかろうじて妨げることが できる、時間の猶予ができるということになります（ただ、ピッタリ時間を止めるとはいいがたい）。

ＧｎＲＨアゴニストだけは、最初の１～４週間あたりでフレアーアップというエストロゲンが異常に分泌されてしまう期間があるので、その間は嚢胞が大きくなったり破裂したりする可能性はあります。

低用量ピルで消退出血をさせる使用法（周期的使用法）の場合、出血期間は若干成長傾向になるかもしれませんが、７日休んで再開すればまた安定します。これを避けるために、４週間ごとの休薬期間ではなく、連続使用する方法でもかまいません（３パックとか６パックとか自分流に。１相性でないとできない）。

◆使用の限度

どのホルモン剤も、デメリット（副作用や後遺症など）によって使用期限が考えられています。ダナゾールの通常使用では４ヶ月まで、ＧｎＲＨアゴニストの通常使用では６ヶ月までです。しかし、黄体ホルモンと低用量ピルにそういう期限は言われたことはなく、５年でも１０年でも薬としてさしたるデメリットはないと言われています。だからといって、低用量ピルや黄体ホルモンだけで内膜症の時間を５年も１０年も猶予できるかと聞かれれば、ノーＮＯと答えます。

デメリット（副作用）というのは、どのホルモン剤でも初日から起こる場合もありますよ。
また、ＧｎＲＨアゴニストの場合、閉経後状態にして病巣を眠らせるという作用があるわけですが、骨が減っていくのも血管が老化するのも同じ作用の範囲です。でも、ねらいからはずれた内容なので、便宜的に副作用と言ってるだけのことです。

脳死という言葉ありますが、脳の働きこそ、人の心のとの関係が

持っているので、脳の正常な働きがなくなった時に、どこからどこまでを人の死としてみるのかという問題を引き起こしている、原因の一つでも
ありますね。私の身内の中にも同じようなことでなくなった
ものがいましたが、心と脳の関係を考えるときに、人の心や、生きるといういうことは、いったいどういったことなのか、考えさせられます。
脳との心の関係が解明されてくるにつれて、今まで信じられてきた

スマートドラッグ（英: smart drugs）は、人間の脳の機能や能力を高めたり、認知能力や記憶力を高める薬品や物質の総称である。スマートドラッグは、狭義には医薬品のみを指すが、広義には栄養素やサプリメントや健康食品などを含めることもある。ここでは、後者の定義を使用する。英語圏では"Smart drug"のほかに、後者の定義と同じ意味で"ヌートロピック"（Nootropics：向知性薬、すなわち脳の認知能の向上薬を指す）という呼称がより多く使われている。略称は、スマドラである。スマートドラッグの日本語訳には「頭のよくなる薬」^[1]、「頭をよくする薬」^[2]などがある。脳の機能を高める栄養素はsmart nutrientsと呼ばれ^[3]、日本語訳には「頭をよくする栄養素」^[4]がある。

スマートドラッグは主に、脳の神経伝達物質や、酵素・ホルモンなどの神経化学物質の供給を増やしたり、酸素供給量を向上させたり、神経の成長を促進させたりすることによって働くといわれている。

また、スマートドラッグと呼ばれる化合物の大半（いわゆるヌートロピック）は、単なる栄養素や植物成分（ハーブ、根、豆、樹皮など）で、一般用医薬品としてドラッグストアで購入可能であり、栄養補助食品として利用されている。 全体のうちのごく一部のスマートドラッグは処方せん医薬品であり、認知学習障害やアルツハイマー病、パーキンソン病の治療や、酸素欠乏による低酸素症を防ぐ目的で使用される。これらの医療用医薬品は人間強化を目的とした様々な応用も可能であるためにインターネット上で大規模に売買がなされており、また、医療目的外の個人的な認知能力の向上を目的とした多くの人々によって使用されている。

なお、専門家による科学的研究はスマートドラッグの利用者が主張する利点の一部を支持する一方で、大半のヌートロピックに関する多くの主張は、科学的に正式なテストが行われていない点には注意が必要である。

神経伝達物質の補助として神経伝達物質の産生に必要な前駆体や補因子を体内に補給する。 なお、心血管を鍛える有酸素運動を定期的に行うことで、心臓が脳へと供給する酸素量を増やすことでも向知性の効果があることには留意すべきである。

日本では、スマートドラッグのうち、処方箋医薬品に当たるものは、処方箋または個人輸入によってのみ手に入り、特に個人輸入がよく使われる。また、一般用医薬品やサプリメント類は日本国内でも手に入るものも少なくないが、アメリカのものに比べ非常に高価で、成分の含有量がとても少ないことが多いので、やはり個人輸入に頼ることが多い。

[編集] 神経伝達物質を調整するスマートドラッグ

思考は、生物学的には難しい部類の作業である。思考はニューロンを活性させる必要があるが、そのためには十分な神経伝達物質を必要とする。この神経伝達物質は、シナプス末端への再取り込みにより再び利用することが可能であるものの、それでも徐々に消耗してしまう。神経伝達物質の消耗は頭の働きのパフォーマンスを低下させ、注意力、論理的思考能力、学習能率、想起能力、調整能力、処理能力、レスポンスなどを悪化させるほか、憂鬱や精神的疲労をも引き起こす。その結果として、仕事や活動においてのミスも多くなる。そしてストレスは、神経伝達物質の劣化をより促進することが知られている。

Well, it finally happened. I received a copyright notice from a publisher about reprints that are posted on my lab website. I actually thought most publishers were sympathetic to the realities of scientific communication and let that sort of thing slide, but apparently not. I'm particularly disappointed that it comes from the National Research Council (NRC) Press of Canada.

Sometimes people would ask what would happen if a publisher asked me to take down my papers. Often I would answer that I would comply, but that I would never have anything to do with that publisher again, either as an author or a reviewer. The fact that this is from the NRC Press is a shame, because I have published a lot with them and have reviewed a large number of manuscripts for several of their journals. I am also working on a special issue for one of them, but I may have to reconsider that now.

Anyway, here's the message. It's polite and not threatening at all, but it does make it clear that colleagues outside of Canada will have to ask me for each reprint that they want, which is a huge pain for them and for me.

Design

Rapidfire Heated Jacket Liner

The first thing you notice when you pick up the Rapidfire Heated jacket liner, is the quality that is built into this product.  It is very thin for a heated jacket liner so it will fit very well under your riding gear.  I removed the winter liner from my Tourmaster Riding Jacket, and replaced it with the RapidFire jacket liner.  This provided a decrease in bulk with a dramatic increase in cold weather comfort.

The liner has stretch panels along the arms, shoulders and sides that help you get a tight fit but remain comfortable with the movements necessary to pilot a motorcycle.  A tight fit is necessary in heated gear as you want the heat source to be close to the body.

The collar of the liner has a very comfortable fleece lining that feels very nice against the neck and it is heated as well.  When zipped to the top, the liner covers all of the neck and has allowed me to ride without the neckgaiter that I have used in the past to keep the wind off my neck in cold weather riding.

The jacket liner is wind and water resistant and has a Polyurethane coated nylon outer shell.  On cooler mornings, where powered heat is not required, the jacket liner does a very nice job without its heat source of providing comfort down in the 40 degree range.  Because the liner is very comfortable, it is not intrusive to be worn as a secondary clothing layer.  Most heated gear is in the way when you don't have the intention of plugging it in.

One of the very nice features of the liner is the zippered pocket along the back of the liner.  This pocket can be opened and the liner stuffed inside to make a nice storage pouch for the liner when it is not needed.

The liner contains nanotube technology to provide heat; making it virtually wire-free.  The torso, neck and sleeves have very small (nanotube) wires running through them that are so minimal you can not feel them unless you really search for them.  Along the waist of the liner and the end of the sleeves, it does have wires that provide connection points to the wiring harness and the glove liners.  These wires are out of the way and do not adversely affect the comfort of wearing the liner.

This article gives the reader a history of four decades worth of thesauri experiment and development, beginning in the 1800s with a definition of thesaurus that reads, "a treasury or storehouse of knowledge." The first thesaurus the authors mention by name was the non-alphabetically arranged "Thesaurus of English Words and Phrases." I think that what the authors refer to as a thesaurus in modern times is really a stand in for controlled vocabulary, subject terms, etc. They give an ISO definition that highlights this view: "the vocabulary of a controlled indexing language."

An example of another type of early thesaurus is closely aligned with my idea of a thesaurus coming into this reading: uniterms. Uniterms were one word thesauri, but they didn't last long for obvious reasons. Think about thermal insulation versus electric insulation. In a uniterm thesaurus these very different ideas would both be expressed by insulation. The frequent occurrence of different meanings for words in different contexts led to the extinction of this type of thesaurus.

It came as a shock to me. I had never had such aspirations. How was I the name they came up with? Could I possibly pull off something so large if I wasn't passionate about doing it? These were my considerations as I was being pursued to run for state legislature. It was 2008, and a group of Democrats always get together to brainstorm ideas for who should run for state congressional seats. Somehow, my name came up. I identify as a Democrat, but I don't identify with polarizing, and if you'd like to know my thoughts on it, click here. The spot had been held by a longtime politician who was now being termed out. The guy I know, Kris, who approached me explained that they thought I would be good because I'm personable, I like public speaking, and I'm fairly well known around town. True, I like talking to people, and the more random the people the better. Also, I do like public speaking quite a bit, and a lot of people do know me, even if I'm known for idiocy on a stage.

I could kind of see why they thought of me. I do like politics and get pretty into it. I was throwing a block party that weekend, which they seemed to think was something a candidate does, even if I was doing it because I love potlucks and yard games, and that party would ultimate end with a downpour and my friend Joe and I in a kayak trying to navigate a flooded ditch by the road.

I was considering it, well, I kept trying to, but every time I did, I just giggled, a lot.

In the past, I've been known to do things that really scare me, in a determined effort to overcome the fear.  Things like climbing Chichen Itza in Mexico; hauling my trembling self up Sigiriya in Sri Lanka even when the panic was really grasping at my mind; walking across a 70ft high rope bridge in Borneo, a bridge that was far to bouncy for my liking (and normally, I'm a fan of bouncy things, like balls, dogs, children).  Yes, I've done a few things that scare me, and last night, I managed to achieve another – SCUBA diving.

I am a bit of a water baby.  I love to be near it, on it, and in it.  I adore swimming, or simply floating to be honest!  Sometimes, I like to swim underwater for a little while, yet I'm never far from the surface, and can easily pop up when I want some air.  The idea of SCUBA diving has never really appealed, simply because I know I'm not too great with having things around my face and neck and eyes.  Therefore, having a mask clamping my nose shut and a plastic tube shoved in my mouth is not in the slightest bit appealing to me.

That was, until a friend of mine suggested diving with sharks as one of my 40 Things for 40 list.  At first, I was horrified at the mere idea.  Not only the diving aspect, but the real reason why the idea of diving (at least in open sea) terrifies me…de dum…de dum….SHAAAAAARK!
